Instructions:

For a facility related EMERGENCY that needs IMMEDIATE attention that can't wait until the next business day, please text David Yell, Property Director, at 919-274-6246 AND then fill out this request.

  • Purchase Requests:
    Please list the Purchase Order number from NetSuite for any repairs and estimates, as well as for any single item $250 and above.
  • Invoices and Receipts:
    Please attach any invoices or receipts to help us better keep track of costs.
  • Reports:
    Please attach any reports related to the facility. (Fire Inspections, State Inspections, etc.)
  • Please include any photos that may help access and/or document the situation.
